Hetuvyadhikar Roga in Ayurveda: Menstruation irregularity associated with type2 Diabetes : A Case Study

Introduction:
Ayurveda acknowledges the concept of Hetuvyadhikar Roga, where a primary disease (Swatantra Vyadhi) may lead to the development of a secondary condition (Paratantra Vyadhi). This case study explores the clinical application of this principle in managing a female patient with hypermenorrhoea and newly diagnosed type 2 diabetes mellitus (T2DM), rooted in the Ayurvedic framework.
Methods:
A 38-year-old female presented with irregular and heavy menstruation, diagnosed as Asrigdara (specifically, Kapha-Pittaja type) and concurrently discovered to have T2DM. Diagnostic work-up included laboratory evaluations and ultrasonography, revealing PCOD morphology. Ayurvedic treatment focused on Dosha Pratyanika and Vyadhi Pratyanika approaches using Vamana (therapeutic emesis) followed by oral administration of Ayurvedic formulations including Avipattikara Churna, Kutki Churna, and Ashokarishta. A Pitta-pacifying diet and lifestyle regimen were also advised.
Results:
Over the span of one year, the patient experienced significant improvement in menstrual regularity and bleeding pattern. Sanitary pad usage decreased from 27–28 pads per cycle to 10 pads. HbA1c levels improved from 12.2% to 8.3%, and fasting glucose reduced from 180 mg/dL to 106.5 mg/dL. The patient’s menstrual cycles normalized within six months of treatment initiation.
Discussion:
The findings affirm the efficacy of treating Asrigdara as the primary disorder, which led to favorable outcomes in secondary diabetes. The interconnectedness of PCOD, menstrual irregularities, and metabolic disturbances reinforces the Ayurvedic doctrine of Nidānarthakara Roga. Holistic management through classical Ayurvedic principles addressed both conditions without isolated targeting, highlighting its clinical relevance in complex metabolic and reproductive disorders.
