Laghumanjisthadi Kashaya and Mahatrinaka Taila in the management of Vicharchika: An Exploratory review article

Abstract
Background: Vicharchikā, described under Kṣudra Kuṣṭha in Ayurveda, is a chronic, relapsing inflammatory skin disorder comparable to eczema. Despite advancements in modern dermatology, long‑term management remains challenging, prompting growing interest in Ayurvedic external therapies. Laghumanjisthadi Kashaya and Mahatrinaka Taila are classical medicines indicated in Kuṣṭha, particularly conditions associated with Kapha predominance, Kleda, and Srāva.
Objective: To critically review the classical composition, Ayurvedic pharmacodynamic attributes, and probable therapeutic role of Laghumanjisthadi Kashaya and Mahatrinaka Taila in the management of Vicharchikā, with correlation to contemporary pharmacological evidence.
Methods: Classical Ayurvedic texts including Bharat Bhaishajya Ratnākara and relevant Nighaṇṭus were reviewed to analyse the Rasa‑Guṇa‑Vīrya‑Vipāka and Karma of the constituent drugs. Published scientific literature was surveyed to identify experimental and pharmacological activities relevant to eczema.
Results: The ingredients of Laghumanjisthadi Kashaya and Mahatrinaka Taila predominantly possess Tikta and Kaṣāya rasa, Laghu guṇa, and Uṣṇa vīrya, which contribute to Kapha–Vāta śamana, Kleda hara, Rakta prasādana, and Tvak doṣa praśamana actions. Contemporary studies demonstrate anti-inflammatory, antimicrobial, antifungal, antioxidant, antipruritic, and wound-healing properties of the constituent drugs, thereby supporting their clinical utility in the management of eczema.
Conclusion: Laghumanjisthadi Kashaya and Mahatrinaka Taila represent a rational, multi-targeted Ayurvedic therapy for Vicharchikā. Their classical attributes and pharmacological actions justify their role in the long-term management of eczema. However, further clinical studies are warranted to substantiate their efficacy and to standardize their application.
